The forms below are split into 3 parts (for ease of reading, we hope!), and they are in alphabetical order. Please let me know if you can't find the form you're looking for.

• Archives Donation Form (AB-Council.13, PDF) - This form needs to be used each time someone makes a donation of material or uniform/insignia to the provincial Archives.

• NEW! Guiders: During the spring cookie campaign, it will be necessary to use one of these two forms for cookies

• Also please note: PRC's from BackCheck will only be accepted if they are received electronically through an iMIS home. For non-Members or PRC renewals the cost to the individual is $24.50 + GST

  • Please note: try to use the Word document if you'd like to fill the form in on your computer and e-mail it to the provincial office (i.e. without having to print it out and then mail it). But, one tip - try to use the print layout in your Word program. Sometimes if you use other layouts (i.e. web layout) things don't line up properly.

  • you can use these Excel forms for unit bookkeeping - they are great because they are linked i.e. do the final adding for you when you do the financial year-end, plus they have electronic monthly bank reconciliations and deposit slips.
